FIRST VISIT FORMS
MEDICAL CONSULT WAITING ROOM

Follow these steps

  1. Fill out the First Visit Forms on the left

  2. Make sure to select Submit at the end of every Form

  3. Enter the Medical Consult Waiting Room on the Left

  4. The medical assistant will greet you to ask you a few questions and make sure your paperwork is complete.

  5. The medical assistant will then transfer you to the waiting room of the Medical Provider. Once you complete the visit with the Medical Provider, you are done!
